Sciatica is a term commonly used to describe pain that travels from the lower back or buttock down into the leg. It usually happens when a nerve in the lower back becomes irritated or compressed. Symptoms can vary from mild discomfort to sharp pain, tingling, numbness or weakness in the leg. At Farrelly Chiropractic, we assess sciatica carefully to help determine what may be causing the pain and whether chiropractic care is appropriate.

What is sciatica?

Sciatica is not a diagnosis in itself. It is a pattern of symptoms caused by irritation of the sciatic nerve or one of the nerve roots that contribute to it. While many people think of sciatica as simply leg pain, true sciatica usually involves nerve-related symptoms such as pain radiating below the knee, tingling, burning, numbness or weakness.

Common symptoms of sciatica

Symptoms of sciatica can include:

  • pain in the lower back, buttock, thigh, calf or foot
  • pain that travels down one leg
  • tingling or pins and needles
  • numbness in part of the leg or foot
  • weakness in the leg
  • pain that is worse when sitting, bending, coughing or sneezing

 

Not all leg pain is sciatica. Some pain can come from muscles, joints or referred pain from the lower back without direct nerve irritation. This is why assessment is important.

What can cause sciatica?

Sciatica may occur for a number of reasons, including:

  • a disc bulge or disc herniation
  • inflammation around a lumbar nerve root
  • narrowing of the spinal canal or the opening where the nerve exits
  • degenerative changes in the lower back
  • irritation of structures around the nerve

In some cases, symptoms settle relatively quickly. In others, they can persist and interfere with walking, sitting, sleeping or normal daily activity.

Does a disc bulge always cause sciatica?

No. Many people have disc bulges on MRI without any pain at all. A scan finding does not automatically mean it is the source of the problem. Sciatica is more likely to occur when a disc bulge, inflammation or another change is actually irritating a nerve. This is why scan results always need to be considered alongside your symptoms and examination findings.

How we assess sciatica

At Farrelly Chiropractic, assessment begins with a detailed history and physical examination. We look at:

  • where the pain travels
  • whether there is tingling, numbness or weakness
  • what movements aggravate or ease the pain
  • how the lower back, pelvis and nerves are functioning
  • whether there are signs that referral or imaging may be needed

 

The aim is to determine whether your symptoms appear consistent with sciatica, what may be contributing to them, and whether conservative care is appropriate.

Can chiropractic care help sciatica?

Chiropractic care may help some people with sciatica, depending on the cause and the severity of the symptoms. Treatment is tailored to the individual and may include gentle hands-on care, movement advice and strategies aimed at reducing mechanical irritation and improving function.

Not every case of sciatica is the same. Some people respond well to conservative care, while others may need imaging, medical management or referral. Careful assessment is important before deciding on the most appropriate approach.

When imaging or referral may be needed

Imaging is not always necessary in the early stages of sciatica, especially if symptoms are mild and improving. However, imaging or referral may be appropriate in some cases, such as:

  • significant or progressive weakness
  • severe or worsening nerve symptoms
  • symptoms that are not improving as expected
  • suspicion of a more serious underlying cause
